MASC Ltd. Group Therapy Participation Agreement
Confidentiality and Privacy
Participation in this group is founded on mutual trust and respect. While facilitators are legally bound to maintain confidentiality, group members are not covered by the same legal protections. Therefore, all participants share the collective responsibility of upholding confidentiality within the group.
MASC Ltd. strictly adheres to professional, legal, and ethical confidentiality guidelines as established by relevant professional organizations and applicable laws.
Confidentiality may be breached only in the following circumstances:
- There is a clear and present danger of harm to oneself or others.
- There is knowledge or suspicion of abuse or neglect involving a minor child or an incapacitated adult.
- Disclosure is required by a court subpoena or other legal mandate.
As a participant, you agree not to disclose any identifying information about fellow group members outside of the group. This includes, but is not limited to, names, physical descriptions, personal details, and discussions that take place during sessions.
Group Participation Expectations
By joining this group, you agree to the following:
1. Commitment to Attendance
- You will attend all scheduled sessions, arrive on time, and stay for the entire duration.
- The group will begin and end at the scheduled time.
- If you are unable to attend a session, you will notify the facilitator at least 24 hours in advance.
- If you fail to provide 24-hour notice, you will be responsible for the full session fee.
- If you miss three sessions, you must meet with the facilitator to discuss your ability to continue in the group. Failure to commit to attendance may result in a pause or discontinuation of participation.
2. Participation and Engagement
- You are encouraged to discuss your therapeutic goals and reasons for attending.
- You will not be forced to share personal experiences before you feel ready.
- Physical contact between group members is strictly prohibited.
- All participants must turn off their cell phones during group sessions.
3. Substance Use and Conduct
- You agree to attend sessions sober and not under the influence of alcohol or controlled substances.
- Food and drinks may be allowed at the facilitator’s discretion.
4. Feedback and Program Evaluation
- Group outcome measures and satisfaction surveys may be conducted. All data will be reported in summary form, ensuring that no individual participant is identifiable. This feedback helps MASC Ltd. improve services and evaluate program effectiveness.
Acknowledgement and Agreement
You confirm that you have carefully read, understood, and agreed to the terms of this policy. You have had the opportunity to ask any questions and receive clarification. You acknowledge that signing this agreement is required for participation in the group. Refusal to sign will result in exclusion from the group.
